Insurance Information: Your insurance is a contract between you and your insurance company. You are responsible for your bill regardless of what your insurance pays. You may request a receipt which should be used for insurance purposes. Our office staff will file insurance claims for gynecological and surgical care. You are required to present our front desk staff with your insurance card at time of visit. If you do not have your insurance card with you, you will be asked to reschedule your appointment. Please be sure to fill in your insurance information correctly, including group and identification numbers. We will help you in whatever way we can, but you can help us save time and serve you better if you make sure that your particular insurance policies cover the services for which you want to file claims. We will assist you making sure that your insurance pays us timely and appropriately for services we have provided to you. However, please be aware that ultimately , you are financially responsible for payment of bills incurred. This office participates with the insurance plans listed below. If you do not see your health care plan listed and you have any questions regarding participation with your health plan, please let us know. We also accept self pay and out of network patients and would be glad to assist you in arranging for payment of services. |
